Lost Birth Certificate | Documentation Station: What You Need To Order a Birth Certificate Today

Ordering a copy of your own or your child’s official birth certificate online is pretty simple and shouldn’t take very much time. Before you order, you want to make sure you have all the details and information you need to make the process go as smoothly and quickly as possible. Luckily, getting a birth certificate doesn’t require you to track down obscure details about yourself or your kid. For security sake, you simply need to prove that you are who you say you are.

Location of Birth

Since there might be multiple people with the same name and birthday, it’s essential that you know where the person was born when ordering a copy of the birth certificate. Some states ask you for the county of birth, if you can’t remember the exact city or town, and some states may even ask for the hospital name.

Important Details

You’ll need some basic information about the person for whom you’re ordering a certificate. While you don’t need his or her Social Security number or blood type, you do need to know the month, day and year of birth, the person’s gender and his or her full name.

When ordering an official birth certificate, you also need to give a reason for the order. The reason helps determine what type of certificate you receive, either short or long form. Typical reasons for an order include applying for a passport, driver’s license or registering for school.

Parental Information

You also need to provide the names of the person’s parents when ordering a birth certificate. While you typically need to know the mother’s first and maiden names, some states provide you the option of not listing the father’s name, if he wasn’t listed on the original certificate or if you don’t know who he is. Some states will ask you if the parents were married when the person was born.

Relationship

For security reasons, you can’t order an official birth certificate for just anyone. You can typically order a copy of your own certificate and official copies of the birth certificates for people you’re related to. When ordering, you need to specify your relationship to the person, whether he or she is your child, your parent, or your sibling.

Address

Depending on the state you’re requesting a birth certificate from, the address you provide for shipping and billing may need to be the same. For example, Pennsylvania requires your billing address, or the address connected to your credit card, to be the same as your shipping address. Other states, such as Massachusetts, will allow you to list separate addresses for your home, billing and shipping. If you decide to have the certificate sent to you using UPS overnight shipping, you can’t list a PO Box as the shipping address.

Credit Card

To order a birth certificate online, you need to pay with a credit card. The name on the credit card should match the name of the person ordering. For security reasons, you’ll also have to provide the three-digit code found on the back of Visa, Discover or MasterCard cards or the four-digit code found on the front of American Express cards.

Document to Verify Your Identity

As you near the end of your order, you may be asked to answer some additional identity questions or perhaps send in an identity verification document, depending upon the requirements set by the state issuing your requested certificate. It’s easy enough for anyone to claim to be related to someone online. This identity verification process requires you to provide concrete proof that you are who you’re claiming to be. Typically, you’ll be asked to provide your Social Security Number and date of birth and/or provide a copy of your driver’s license, passport, or other valid photo identification to prove your identity, or a sworn statement proving your relationship to the person whose certificate you’re ordering. These documents can easily be submitted via a scan and upload process, or by faxing if that’s more convenient.

  5. I have a short form birth certificate, but I would like to get a long form official birth certificate from 1960, so what do I put on the line for why I want my birth certificate, so I can get the long one?

    1. Great question – typically long-form birth certificates are provided for a variety of legal purposes, including obtaining a passport. If you need the long-form certificate, we suggest selecting the appropriate reason code when ordering, and you can check during the order process to ensure you’re getting the long-form certificate as well. Thank you.

  6. I ordered and paid for two original birth certificates from Pennsylvania, with parents signatures and doctor signature. I believe that you call them a “book” type. I received a different kind. Please would you help me. Thank you,Cathryn Ada Moss

  7. I was in the process of ordering several copies of my late husband’s death certificate. I mostly completed the process, including typing in my credit card info, until I arrived at the final page which requested MY SOCIAL SECURITY NUMBER. This is unacceptable. I could think of no reason whatsoever as to why you would be requesting this information. Needless to say I bailed on your website.

    1. Thank you for your comment. Your Social Security Number was requested as part of our online identity verification process, to help ensure the certificate being requested is only being ordered by persons who are authorized to do so. This is done as a secure process when ordering remotely, to satisfy the identity verification requirement from the Vital Record Agency issuing the requested certificate. If you have any additional questions, please feel free to contact us at Vitals.Research@vitalchek.com. Thank you.

  8. I need to order a birth certificate but it is in my maiden name. When I sign the agreement form it rejects it because the name does not match. Do I only use my maiden name?

    1. Great question. When applying online for your birth certificate, when it asks for the name on the certificate you should use your Maiden name (or the name that was used on the birth certificate). In other areas of the ordering process, you can use your current legal name. If you have any additional questions, please reach out to us at Vitals.Research@vitalchek.com and we’ll be glad to assist. Thank you.

  12. I signed up for my baby birth certificate at the hospital and checked the “yes” box to receive a ss card. Will the ss card come if I haven’t paid for the birth certificate yet?

    1. Thank you for your question, and congratulations. Generally, if you’ve requested the ss card at the same time you signed up for your baby’s birth certificate, the hospital will provide that information to the appropriate State agency to issue the birth certificate, who in turn will provide your child’s information with the Social Security Administration to then mail the ss card to you. If you have any questions, it’s best to visit the Social Security Administration’s website at http://www.ssa.gov. Thank you.

  26. I find it quite cumbersome. San Bernardino requires you to insert your ID picture on their PDF file, and I have been trying to do so for the last hour without success. I just added another page with my ID picture, hoping that it will be acceptable. Riverside County, on the other hand, is far easier. I understand the verification process is needed, but it could be made a bit smoother, considering I already paid over $100.00 for two birth certificates. My goodness.

    1. Hello and thank you for your feedback.

    1. Hello and thank you for your question. Each state handles the letter of exemplification or apostille a little differently. When choosing international marriage most states will then ask if you also need the apostille/letter of exemplification, be sure to say yes. This can sometimes be a 2 part process due to some locations having to send the records to the secretary of state for the additional certification. To find out the process in the state you were born in you can contact that vital records office. You can also contact our office at 1-800-255-2414.
      Thank you.

  30. My grandmother was born in El Paso, Tx, she moved to Chihuahua Mexico, where she lived undocumented her whole life, she tried to request a birth certificate from the state of Texas many times, she did have a baptism record from the church in texas, i did request an original which i do have, her baptism copy has a vital statistics stamp from the state of Texas, I do have the signed original delayed request for signed by her and the returned mail from various offices, she died a few years ago and I just wanted to honor her wish of obtaining her birth certificate,it was my understanding the one never loses her U.S citizenship, is there a way I can request it?
    please help.

    1. Good morning Ruben And thank you for your question. The state of Texas has a list of entitle people who can order a birth certificates. The list of entitle people are as follows: Self (your own certificate), Mother, Father, Spouse, Adult Child, Adult Sibling, Maternal Grandparent, Paternal Grandparent, Legal Guardian.\This would mean that you, being the grandchild are not entitle to order her certificate but she would have been able to order yours. We would recommend you contact the Texas state office or El Paso County Clerk directly to see if there is any way to bypass this. We wish you the best in your journey to fulfill your Grandmothers wishes.
      Thank you,
      Nichole H.

  31. Hi, I really appreciate your help with this question. I have a friend born in Virginia, now lives in Georgia. The persons ID was stolen and since then her mother and father have passed and she is unable to get in contact with any relatives, we have tried to order a birth certificate, and provided all the necessary information yet she was still denied because they said you need ID as proof, which was the only reason she was even trying to get a birth certificate, to get a new id? Wow. I mean really, can you please give me some advice here. Situation seems impossible. She is not married, can’t get in contact with her brother, the only living direct family member.

    1. Hello Joseph and thank you for your question.
For the state of VA when ordering online through http://www.VitalChek.com we have an online verification process for the protection of the customer. Virginia needs to be sure that the person order the record is in fact who they say they are. If this person fails the online verification then they are asked to upload or fax in proof of identity. If this person does not have a valid state issued i.d. VA will accept 2 “alternate i.d.’s”.
Acceptable alternate Forms of identification for the state of Virginia (TWO of the following:)

      – US Selective Service card – Social Services Identification card
      – DD-214 Form – Pilot’s License
      – Certified School Transcript – Weapons or Gun Permit
      – Health Insurance card – I-797 form
      – Life Insurance Policy – IAP-66 form
      – Welfare Identification card – Veterans Universal Access ID card
Hopefully your friend will have at least 2 items from this list. If not please have your friend give us a call so that we may assist further.

      Thank you,
      Nichole H.

  32. Nicole, I really appreciate your reply more than you know. We keep getting stuck on ”situation impossible” since her parents are both deceased. The only living direct family member is her brother, and he is not responding to her or my requests to help at all. The poor girl had her identity stolen and we just can’t seem to get anywhere, even my mother tried to order a birth certificate for her from another site and ultimately she was turned down, even though she provided every single minute detail about all her deceased (mother and father) and all her personal information down to the letter. It’s really been frustrating, especially for her. She is literally in tears over this now because she got a special grant to get help with all she has been through and fly to California, but now can’t even go because no ID. That seems like the only thing we actually can verify, all this is true I could have you talk to the agent Jacob Mouradian personally. Also I left this bit of information out in my initial post because I did not think it would help because he has been no help, in fact refuses to help her and keep her in the abusive situation, rather than help at all. So not sure what to do, she has none of those documents. Situation has become increasingly frustrating, and all the poor girl needs is her id. I am typing all this because I am trying to really dig, she knows I posted this now, you seemed to be understanding in your response but I could have her call you. She really needs help here, we are really trying to help her get her id.

    1. Typo on that, my goodness. Also I left this bit of information out in my initial post because I did not think it would help because he has been no help, she in fact married, but refuses to help her and ”rather” her in the abusive situation, rather than help at all.

    2. Jacob,
If she doesn’t have any of those alternate forms of I.D. then I was asking that she call our office. The reason I ask for her to call our office is to see if she can possibly pass the verification over the phone. IF she passes her online verification then no I.D. is needed.
It was very nice of your mother to offer to order the certificate for her however only entitle people can order her certificate.
Here is the list of entitle people in the state of VA:
◦Self, Father, Mother, Brother, Daughter, Husband, Sister, Wife, Son, Legal Guardian, Maternal Grandparent, Paternal Grandparent

Has your friend explained this situation to the state of Virgina directly? Since these are the rules of the state they are the people that ultimately have the final say so.
As a side note- It concerns me that you have tried to order from other online services since we are the actually the authorized provider for Virginia. With that being said, please be careful when visiting the unauthorized sites.

Wishing you and your friend the best of luck.
      Thank you,
      Nichole H.

  33. Nichole, sincerely once again thank you for replying and your advice. As far as the online company, she filled out the form down to the letter, and my mother paid for it. The company we tried to get it through was usvitalrecords. We got all excited because finally, there seemed to be some hope for her to move on with her life, get insurance, file for disability etc, but then they sent a letter requesting PROOF OF ID. Of course, the only reason we were trying to get her birth certificate in the first place was to get her ID. This was several months ago, I believe we tried to call them but to no avail, I remember they made a suggestion that one of her relatives ie: brother, spouse, mother, father etc, or someone she is married could help prove it was her, but unfortunately for her, everyone has passed besides her brother, and he just does not even seem to care no matter how much she or i try to get through to him of how important this is for her.

I believe I will have her call vitalchek, actually do you have an agent number so she could possibly speak to you. Today we went to her a former lawyers she had gotten a claim for after an automobile accident named Deliah Green here in GA, they seem to really get along I was there, she not only gave her the file folder (lots of documents) to try to help her secure her birth certificate, but to my own surprise she went so far as giving her the actual folder. Said she would be willing to notarize something in case she needed it.

She does have a daughter but I do not believe they are close at all, unfortunately things like this happens but no matter how bad someone’s life might seem, I believe they still deserve a chance to make it and get help and continue with their lives somehow. Right now the girl is so fed up she does not even want to continue, but at the same time felt there was a chance she could make it out once this person Jacob and the foundation he works approved her, by speaking with her. He knows how much is going on, and I can’t get into that much here, but I really hope she can do it through vitalchek SOMEHOW.. So many let downs aside from her getting her birth certificate, I really think she deserves a chance. She is willing but very very depressed, all I can do as a friend is try to dig. Is it possible you could give me the number to vitalcheck so I can have her call you or someone in your agency she could possibly get an over the phone approval for. Me or my mom are willing to pay the cost for her. Thank you for enduring my lengthy post, like so much.

By the way, we thought there might be a copy of her ID on file at her lawyers, but to our surprise there was none. Folders filled with papers but no copy of her ID. Not really sure what good that would have done. Thank you! Praying for a miracle, she is ”supposed” to leave for CA on the 1st of December according to the agent in CA which said she had somehow been approved for rehab, he has her checking in with him, everyday.

    1. Good morning Joseph,

Please have your friend contact our office at 1-800-255-24142 ext 86885 (Debra) we will be more than happy to help her place an order and hopefully your friend will be able to pass her online verification. When she calls be sure to let the agent know the situation and that it is very important that she pass her verification. Wishing you all the best.
      Sincerely,
      Nichole H.

  34. Nichole, It’s difficult to express with words how thankful we are for you and Debra for helping us. In fact, it is the only reason I have not done so sooner.. I really feel a simple thank you, to the personnel at Vitalchek alone would not be enough. We are overjoyed, I am beyond thankful that Vitalchek was able to help secure Holly’s birth certificate. It’s not just that, you both helped change someone’s life when the situation seemed impossible.

The staff at Vitalchek is unlike our experience with another company that turned us down flat. I remember her giving up hope again and crying, and just wanted literally to give up being that, not only is she the lone survivor of her family but that all she needed was someone out there who really seemed to understand. I really been praying about this, I never even expected a reply, I just want her to be able to have the chance to move on, and all I can say is when nobody else, and everyone else turned her away Vitalchek was there.

It might seem a little over the top, but I, we will never forget this moment. Thank you for your help, taking the time not only to answer our/my questions but taking the time to help us understand. If by chance someone is out there and you read this know, even if your situation seemed impossible and feel like giving up, don’t. Don’t give up. Pray, there is a way. I highly recommend this company.

    1. Joseph,
Thank you so much for taking the time to write this. I am truly happy that we were able to help you and your friend. That is such wonderful news. You make a great point, Never give up! Joseph, I will be sharing this thank you message with our entire VitalChek team.
It’s been a pleasure assisting you.
      Sincerely,
      Nichole H.

  35. Hi, my brother doesn’t know the city, county, or hospital he was born in. How does he find out?

    1. Hello Jennifer- Depending on the state that your brother was born in it may not be as big of a deal as most would think. Some states can perform a state wide search. To find out if the state where your brother was born can perform this state wide search please have him contact our office at 1-800-255-2414. We will be happy to assist.
      Thank you,
      Nichole H.

  36. My order has been cancelled because it states that, after my third time faxing in my documents, that its still not being processed. I’m not to sure where and who needs to notorise my documents please give me an answer. I’ve been going back and forth for a month now.

    1. Good morning Aubree- If you would please send us an email including your VitalChek order number to Vitals.Research@vitalchek.com or call our office at 1-800-255-2414 . We’ll be happy to look at your order and find out what is going on.
      Thank you,
      Nichole H.

  37. Hi,
pls I just put to birth with a new baby, I want to know after how many days I can get an expedited birth certificate in nyc.thanks

    1. Hello and thank you for your question. In New York City it takes up-to 2 weeks (typically means 14 business days) for a new birth to become registered.
      Thank you,
      Nichole H.

  38. My son was born in Los Angeles County four years ago at home, and has never been to a hospital, as we had a midwife. Shortly thereafter, we moved out of state. So a Birth Certificate was never issued. His two older sisters have them, and we have the paperwork the midwife filled out. Any advice?

    1. Hello Matthew & thank you for your question.
Since you are unsure if you provided the county with the birth registration information the best place to start is with Los Angels county vital records. Explain your situation and ask them what you need to do to register your son if he is not already registered. You can reach their office at 562-466-2199.
      Hopefully this information will help.
      Sincerely,
      Nichole H.

  39. Just applied and paid for a replacement birth certificate. Not sure if I need to send a verification document for my order. If I do I also need a form- indicated by the faxing instructions. How do I find out if I need this verification or not?

    1. Great question, thank you. If you ordered your replacement birth certificate through VitalChek.com, we will provide you with a follow-up email if any necessary additional documentation is required. If you wish to check, simply log-in to the Manage My Order tab on VitalChek.com with the order # and PIN # provided in your email confirmation, and any required instructions will be provided there as well. Thank you.

  40. HELLO. I’m currently trying to get my g.e.d and start college and all kinds of stuff. I’m 21 y.o and been through alot the last 8 years. Due to that, when I was 18 my birth certificate and social security card were stolen and I had and have never had an identification card our license so how do I go about getting my SSC and birth certificate in Arizona. Cause I need those to get an ID but I also need an ID to get those. So what do I do? I’m finally learning to get back on my feet and want to do something with my life and everything depends on me getting an ID OR LICENSE. LITERALLY. PLEASE HELP ASAP.THANK YOU.

    1. Thank you for your questions. Believe it or not, there are a lot of people in similar situations where they don’t have ID to get a certificate, but need a certificate to get ID. The best thing to do in this situation is to contact the vital record office where you were born (generally the health department or county clerk’s office), and talk to them about your situation. They should be able to provide you with an alternate process, using other forms of identification, so you can obtain your birth certificate in order to get your ID. Thank you.

  41. Hello,

I am trying to help my girlfriend out. She was born in Pottstown, Pennsylvania. A few years ago, an ex boyfriend of hers burnt all her identification papers. She wishes to get her state id for North Carolina, but because she doesnt have her papers, they wont let her do it. We are going to be getting her Social Security Card from the Social Security Office sometime this coming week. We still need her birth certificate, but from what I’m told, she cant get that with out I.d. and Social Security Card. How can we go about getting this for her, cause she needs the certificate and the I.d. badly. Any help would be appreciated. Thank you.

    1. Thank you for your question. We’d be glad to help, just email us at Vitals.Research@vitalchek.com, and one of our representatives will be glad to assist you with your questions. Thank you.

      1. I appreciate the response. Unfortunately, she is on SIS, and doesnt have a bank card or credit card. If it would have been permitted, my mother or I would have used one of our cards to pay for it.

  42. I had a baby on 7/27/15 in NC, after being separated from a previous marriage for almost 3 years my divorce was final in April of 2015. I have court documentation showing my ex-husband is not the father of my baby, however when I filled the form out in the hospital because my divorce wasn’t final for more than 220 days I could not list my boyfriend as the baby’s father. How do I get him listed on the birth certificate? I have called the NC department of vital records and they say I have to get a court order for this which makes no sense being that I have a court order signed by a judge showing my divorce was final and my ex-husband is not the father of my baby. Please help!!!!

    1. Thank you for your question. It sounds like you’ve already spoken with the proper agency (the NC Department of Vital Records) to get an answer to your question. You could try contacting that agency again to see if there are any alternate methods, rather than a court order, to get the change you are requesting. Since only the vital record agency can determine what documentation is needed to make changes to a birth certificate, it doesn’t sound like we will be able to assist you in this matter. Best Regards.

  43. I have a question. I am trying to help a family member out who is unable to pay for his birth certificate. He was born in NY but currently lives in Florida. I am trying to order a birth certificate for him online. I live in NY. Can he email me a scanned notarized letter authorizing me to order it along with copies of his id?

    thank you

    1. Thank you for your question. Typically birth certificates may be ordered by immediate family members; however, each agency has different rules as to who may order these sensitive documents. To better assist with your specific situation, please email our expert research team at Vitals.Research@vitalchek.com, and they will be able to answer your question directly. Thank you.

  44. Hi,
I have applied to get a copy of my birth certificate but had my request voided not once but twice.
I lost my original birth certificate and need it to apply for a pass board. It says my first name doesnt match.
Not sure why. Please tell me how else I can get a copy.
    Thanks

    1. Hello Joanne and thank you for reaching out.
Please contact our office at 1-800-255-2414 and we will be more than happy to look into this for you. If you have a VitalChek order number please have it when calling in so we can look directly at your order.

  46. Ok, my sister has been trying for like 2 years now, to get my moms birth certificate and keeps being told that they can’t find the file. So what can be done to get a birth certificate for a person that has other records yet no proof she was even born? She needs this in order to get help with housing for the elderly and without a birth certificate she can not get the other things she needs to get approved for help with this housing? Help!!! The house she is living in is not an appropriate place for her and her wheel chair needs. Can somebody please help?
    Thank you.

    1. Hello Edna and thank you for your question.
If she has been trying for 2 years and they have not been able to locate a match you may want to start looking into the issue a little deeper. Was your mother ever adopted? Has she ever had a name change? Is she trying to order from the correct location? Unfortunately this is something that happens more frequently with the elderly. Being born with one name and then using a nickname for everything (including social security cards, ID’s etc.) was very common back then. After asking some more questions I would contact the vital records office in the state where she was born to see if they can assist you in finding it or on how to create one for her.
We hope you are able to get this resolved. Let us know if you need any contact information for the state offices and we’ll be happy to provide them for you.
Thank you and we wish you the best of luck,
      Nichole H.

  47. I’ve lost both my birth certificate and my social security card. How can I get my birth cirtificate if I don’t no, or have my social security number?

      Thank you,
      Nichole H.

  48. Am I able to order a birth certificate again if my first order was canceled?
My Order was put on hold because I did not have proper identification and it took longer than 15 days to be able to have it.

    1. Hello Renee and Thank you for your question.
If you have an order with us that was on hold most likely we can juts reinstate the order for you so you will not have to start over. Please call our office at 1-800-255-2414 and provide us with your order number.
      Thank you,
      Nichole H.

  49. My birth certificate is in Mexico and I’m in Illinois, there’s no way I’m getting it back. At least not anytime, which is bad because it’s urgent. I need it to get a passport. Can you help? Please and thank you.

    1. Hello and thank you for your question.
When you say your birth certificate is in Mexico do you mean that you were born in Mexico? If so are you a US Citizen born abroad (born to at least 1 US Citizen)? If you are then VitalChek can assist you. https://www.vitalchek.com/content/bornabroad.aspx However if you are not then you will need to contact the Mexican Embassy’s office.
      Thank you,
      Nichole H.

  50. Hi,

I need a Puerto Rico birth certificate. I tried getting it through their direct website but since the only form of id that I have is an expired drivers licence they wouldn’t accept it. Will the same rules apply if I do it through vital check or are there any other documents that I can provide as proof of identity?

    Thank you

    1. Thank you for your question. The only forms of ID accepted by Puerto Rico, whether ordering from their website direct or through VitalChek, is a valid Government-Issued Photo Identification (such as a Driver’s License – can be expired up to 30 days, but not beyond), a valid passport (must be current and not expired), or a US Military ID w/photo and signature. Unfortunately they don’t accept any alternates to these ID documents; however, if you have an immediate family member who is entitled to order your certificate (such as your Mother, Father, or an adult child over 18) they may be able to order your certificate for you. Hope this information is helpful. Thank you.

  51. I am trying to upload my documents with secondary identification but what Identity Verification Form are they speaking of ?

    1. Hello Arleni and thank you for reaching out to us. Typically the identity verification form is at the end of your order. ITs a form that you print out. You can access it by going to the manage my order tab on the VitalChek home page https://www.vitalchek.com/. You will thank enter your order number and pin. If there are any required documents for that state they will be here.
If you have any trouble accessing this form please feel free to contact our office at 1-800-255-2414 or you can email us at vitals.research@vitalchek.com.
      Thank you,
      Nichole H.

  52. Will this give me a copy I can print to give to my employer if I ordered it today?

    1. Hello Dominique and thank you for your question.
No this will not give you a copy that you can print out. When you order on the VitalChek.com website you are placing an order for a certified vital record from the state or county vital records office. They will receive your order and process the request then ship the certified birth certificate to you directly.
      Thank you,
      Nichole H.

  53. I need to fax or email my ID card. Don’t know where to send them to

    1. Thank you for your question. If you need to submit additional documentation for an order placed on VitalChek.com, the appropriate fax number or instructions on how to scan and upload the required documents will be shown on the Identity Verification Form you should have received. If you need another copy of this form, you can log-in to our Manage My Order page using your Order Number and PIN Number. Thank you.

  54. I’m trying to get my child’s birth certificate for the first time but he is 3 years old. His dad and me are not married yet so what can I do? That was the reason why I was not able to get it when he was born because his dad was not able to sign paper in the hospital.

    1. Hello Alex and thank you for your question.
Your child’s certificate should be registered in the state that he was born with or without the fathers information. You can submit an application to the vital records office via mail or you can contact us at 1-800-255-2414 or https://www.vitalchek.com/, we’ll see if we can assist. We work directly with hundreds of government agencies nationwide to provide consumers with the ability to securely order their vital records online.
      Thank you,
      Nichole H.

  55. Hello,

My boyfriend and I plan on getting married. I was looking at his birth certificate and noticed that his first and middle names are not listed on his birth certificate just his last name. Is that gonna be a problem you think. I’ve never seen a birth certificate missing information like this. Any information you can give me would be greatly appreciated.

    1. Hello Liz and thank you for your question.
Your boyfriend will need to contact the vital records amendments and corrections department. There are many reason why it could be listed this way. He will need to get the certificate amended to reflect his actual name. If you need assistance locating the vital records office phone number I’ll be happy to assist.
      Thank you,
      Nichole H

  56. Hello,

I sent for a new birth certificate back in 2010. I put employment as the reason for the certificate, because that was why I needed a fresh one. I applied for a passport recently and it was denied. When I ordered the certificate I had know idea there was a long and short form. This should be stated as you are ordering about long and short forms. I had to spend another $30.00 for a new birth certificate. Why do they have long and short forms? This is ridiculous!!!! Why wouldn’t everyone want the real deal. Please explain this. I feel I should not have to pay for this one. I sent for a birth certificate and I should get the whole thing.

    Completely Disgusted
    Richard D.

    1. Hello Mr. Davidowski,
Thank you for reaching out to us. We would like to apologize for the inconvenience that you are experiencing.
When you say you sent for a new birth certificate, do you mean you sent an application in the mail to the vital records office or you ordered through the VitalChek.com website placing an expedited order? I can tell you that not every state has a long and a short form certificate. However both types of certificates are certified from the state or county office. The states that do will typically issue the certificate that is sufficient for the reason indicated on the application. Some states that have different certificate types will give you the option to choose “long form” as the reason for ordering.
If you recall ordering your certificate through VitalChek.com you can contact our office (1-800-255-2414 or Vitals.Research@Vitalchek.com) and we can try to pull up your order from 2010 and see if there is anything that we can do on this end. If you ordered it through the mail then you can contact the state office directly and see if they may be able to assist you with obtaining the copy that you wanted in the 1st place.
      Sincerely,
      Nichole H.

  57. Can I use my none notarized copy of my birth certificate to receive my notarized copy of my birth certificate?

    1. Hello Sasha and thank you for your question. Typically to obtain your birth certificate you’ll identification to show you are who you say you are. Of course this is a security measure to make sure that the vital records office is not giving your certificate away to the wrong person. Depending on the state you were born they may have alternate forms of Identification that you can use. Each state is different with their own set of acceptable forms of identification. If you would like to contact our office to find the rules for the state you were born in you can contact our office at 1-800-255-2414 and we’ll be happy to assist.
      Thank you,
      Nichole H.

  58. Hello, I ordered my long birth certificate, because I need to get my passport and I uploaded the Indentity verification form with a copy of my ID of Texas, and the company sent me an email saying that my order was on hold because you invest receive a good proof of identity, why ? 59. Hello… We are in the process of disputing my brother’s exaggerated claim of inheritance he was denied from mineral rights in Oklahoma, and in order to produce an accurate valuation, we must show proof of my grandmother’s death to factor in what share of ownership was passed on to her heirs. I have ordered a copy of my grandmother’s death certificate from Oklahoma, and have been asked to provide legal documents that verify my entitlement. I am unclear how to prove that I am a grandchild. Of course, my birth certificate includes the names of my mother and father, but am I then expected to have a copy of my father’s birth certificate to show that my grandmother was his mother? Neither my grandmother or father had a will, nor are there any court orders on record that document my lineage. I’m not sure what to do next and would appreciate your advice.

    1. Hello Ruth and thank you for your question. Oklahoma has a very strict verification of entitlement process. Without actually seeing your order I cannot be exact on what you will need to provide however here is what Oklahoma tells us is needed and for who.
Applicant’s relationship: mother, father, son, daughter, wife, husband, sibling, legal guardian, maternal grandparent, paternal grandparent, grandchild, relative, funeral director, authorized rep, court appointed individual, other. Documentation Required: legal documents to verify entitlement to the record requested

With that being said, you should be able to provide documents proving you have a legal need for the certificate along with your identification. If you placed your order with VitalChek’s online service for Oklahoma you can contact our office at 1-800-255-2414 and we’ll be happy to look at your specific order to determine what you should provide.
      Thank you,
      Nichole H.

  60. I have a friend living in Mexico and he wants me to request his birth certificate what paper work do we need to be able to request the certificate? And would I be able to request it?

    1. I have all his information he needs the certificate to apostille as he wants to get married in Mexico

    2. Hello Alejandro and thank you for your question.
To order a birth certificate you must be entitle to the record. Most states will not allow a friend to order another person’s certificate. To determine what paperwork is needed we will need to know what state your friend was born in because each state requires different documentation. To find out more about ordering a certified birth certificate you can contact our office at 1-800-255-2414.
      Thank you,
      Nichole H.

  61. Hello, I am searching for information that will help my mother obtain her birth certificate. Here is the story. My grandmother and my grandfather lived in Wilmington, NC, and traveled to Virginia for work. My grandmother at the time was pregnant with my mother. My grandmother goes into labor and a midwife delivers my mother (not in a hospital). The midwife was suppose to register my mother, and my grandparents traveled back home to Wilmington. My grandmother never obtain a copy of my mother birth certificate. Years ago, due to my mother asking for her birth certificate, my grandmother made a call and was told the place that the midwife stated my mother was registered at burned down, therefore there is no proof that my mother was actually registered. The midwife have passed away, so no other questions can be asked. This has been going on for many years now. All my mother keeps getting is the run around, and lawyers wanting thousands of dollars, in which they are not reassuring. How can my mother get a copy of her birth certificate or proof that she was born.

    1. Hello Sherri and thank you for your question.
This would be something to ask the Vital Records corrections and amendments office in Virginia and North Carolina. Explain to them your situation. They will be able to look for any records and IF they do not have them they can tell you how to get one created for you mother. Of course it would need to be created in the state she was born but you can have both state look for the record “just in case”.
      North Carolina 919-792-5986
      Virginia 804-662-6200
      Thank you,
      Nichole H.

  64. I am trying to get my daughter’s birth certificate quickly. All of our most important documents were either lost/stolen. We moved recently so my driver’s license address doesn’t match my current address. Is it okay to fill out the form that say I do not have a government issued photo-ID and that I am presenting the attached two documents? I do have a government id—just the wrong address. Any help would greatly be appreciated!!

    1. Hello Laurie and thank you for your question.
The I.D. requirements vary from state to state and county to county. In order to answer your question correctly we would need to know where your daughter was born so we may check that locations rules. Please feel free to contact our office at 1-800-255-2414 and we will be happy to assist.
      Thank you again,
      Nichole H.

      1. Pennsylvania* Thank you so much!!

        1. No Problem at ALL Laurie! – Here is what I have for Pennsylvania when ordering through our authorized VitalChek service:

If the customer does not have acceptable Photo ID:
Customer must submit a Statement From Requestors form http://pciutility.risk.regn.net/manuals/StateManuals/forms/Statement_from_Requestors.pdf , completed fully and with documentation specified on the form or as mentioned below.
Acceptable documents that can be provided with this form are a utility bill (Cable bill or Cell/Landline bills count as a utility), car registration, pay stub, bank or credit card statement (2 different bank statements from 2 different banks is acceptable), copy of income tax return/W-2 form, or lease/rental agreement. If your mailing address is a P.O. Box, one of the documents submitted must reflect that you have used the P.O. Box for at least two months.
Of course if you have any further questions you can contact our office directly at 1-800-255-2414. Also, PA has a little higher than normal processing time even though they consider VitalChek orders to be priority/expedited. If you do place the order through VitalChek.com let the representative know that you are needing it urgently so they can possibly send a note to PA asking to rush your order.
Hopefully this information will help you.
          Thank you,
          Nichole H.

  65. Hi I HAVE A Question when I had my baby October of 2015 I filled the paper work to get his social security and birth certificate and I gave it to the nurse when I was done with the paper work I left home and it’s been 7 months and I called the hospital and the told me they had no paper work of my child the nurse did not turn them in Idk what to do now how can I get his birth certificate and social security if the nurse didn’t turn in his paper work ??

    1. Hello Liz and thank you for your question.
I would be concerned if the hospital gave you this information and no resolution to the problem. Did they say that they planned on correcting this mistake? Just to be sure you may want to contact the Vital records office in the state that your baby was born and let them know what the hospital told you. It could be that the person you spoke to at the hospital was unsure of where to look or what the procedures are, it happens.
If you need assistance locating the number to the vital records office please let us know and we will be happy to provide you with their direct number.
      Thank you,
      Nichole H.

  66. Hello

I’m trying to get a birth certificate for my daughter but I don’t think she ever had one. We received a proof of birth when we left the hospital but I don’t think I ever mailed in the birth certificate application. How can I apply for one? I don’t think she has one in the system.

    1. Hello Rita and thank you for your question.
If you received a proof of birth most likely there is a certificate on file at the vital records office. Most state and county vital records office’s have the application available on their government website for you to print off and mail in with a check or money order. If you need assistance locating the direct link to the government site please feel free to reply back with the state and city your daughter was born in and we’ll be happy to provide that for you. You can also use the VitalChek.com service, we at VitalChek work directly with hundreds of government agencies nationwide to provide consumers with the ability to securely order their vital records online.
Please let us know if we can assist further.
      Sincerely,
      Nichole H.

  67. What would be required for a parent not listed on the birth certificate to obtain. The mother is now deceased.

    1. Hello Stacey and thank you for your question.
For most states but not all, if the parent is trying to order a certificate but they are not listed on the certificate they would not be considered an entitle person to receive that certificate. However, some states will allow someone that is not on the birth certificate to order if they are able to provide a specific legal need for the certificiate. To find out the rules for the state you are referencing you can contact the vital records office where that person was born. You can also contact our office at 1-800-255-2414 and we’ll be happy to look the information up for you.
      Thank you,
      Nichole H.

  71. So I’m a young parent and I entrusted my son’so birth certificate to my parents. They lost it after I gave it to them. We are going to be going to a federal prison to see my son’s grandfather they need his original birth certificate. I don’t know how to go about getting another one or even what website to use to get another one.

    1. Good morning Kathy and thank you for your questions. You have a few different options to obtain your son’s birth certificate, if you still live in the same area where he was born you can usually visit the Vital Records office and use their walk in/counter service. The walk in service is usually a same day service and the least expensive route. Please let us know if you need the address for the specific location. You can submit an application via mail in service, the vital records office will have applications on their official website for you to download or print off (there is no fee to print or download the application on their websites), you would then mail the application to their office with a check or money order. The processing time is usually a little higher when submitting your application via mail. Then you have the VitalChek.com option, we work directly with hundreds of government agencies nationwide to provide consumers with the ability to securely order their vital records online. Sincerely,

  72. Hello, me and my identical twin sister both ordered birth certificates from Lowell, Massachusetts so that we can get our passports. When our certificates came, my sister’s looked like a birth certificate and it included which twin she was, however, mine came and it simply a notarized piece of paper with mother’s name and the hospital i was born in. Needless to say when we went to get our passports, I was unable to get mine, as they did not believe it was real. Why is mine different and what can I do to get a real birth certificate?

    1. Hello Joanna and thank you for reaching out to us.
Did you order your certificate through our VitalChek service or directly through Massachusetts? If you ordered through VitalChek please contact our office at 1-800-255-2414 and we’ll be happy to get in contact with the state office and find out why your certificate is different than your sisters. If you ordered your certificate from the state of Massachusetts directly yo can contact their office at (617) 740-2600.
      Thank you,
      Nichole H.

  73. Hy, I was born in South Africa and my mother died after she gave birth to me so I was raised by my uncle, and started my school here in SA but I dont have a birth certificate as it is said I was born at home. So I want to know for which procedure must I follow to apply for a birth certificate?

    1. Hello Tankiso and thank you for your question.
We at VitalChek are not familiar with the procedures in South Africa however we can tell you the best place to start is the South African Consulate or Embassy.
We wish you the best of luck in obtaining this important document.
      Sincerely,
      Nichole H.

  74. I am trying to obtain my birth certificate for a passport. I was born in Wellsville, N.Y. However, I was adopted by my step father at the age of three. I do not have adoption papers either. I know the hospital I was born in and my biological fathers name. Is it possible for me to obtain a birth certificate?

    1. Hello Karen and thank you for your question.
If you were born in the state of New York and adopted you can still order your certificate from the state of New York. You will not need any of the information from before the adoption took place due to the fact when a legal adoption takes place a new birth certificate is created listing all of the adopted information, such as the adopted parents & your adopted name. The old certificate is no longer available to order or considered valid. To order your certified birth certificate you a have a few different options.
If you still live in the New York State area you have the option to use the in-person service located at 800 North Pearl Street, 2nd Floor, Menands, NY 12204.
If the in-person service is not convenient you can submit an application via mail in application. You can visit the New York State website, print off the application, fill it out completely and then mail it back to their office with your payment included. http://www.health.ny.gov/forms/doh-4380.pdf
Of course you can also order your certificate through our VitalChek service via phone 1-800-255-2414 or online application as mentioned by the state of New York on their website http://www.health.ny.gov/vital_records/vitalchek.htm
hopefully this information has been helpful.
Thank you and have a great day.
      Nichole H.

  76. I ordered a NYC Birth Certificate with the reason of Dual Citizen/Immigration I think – would this have given me a long form certificate / letter of exemplification? I need to get an apostille – so need the letter of exemplification/everything necessary for that but not sure if this reason supplies that?

    Thank you!
    Amanda

    1. Hello Amanda,

If you indicated the reason was for Dual Citizen/Immigration then yes, New York City says they will provide you with the appropriate certificate, “Birth Vault Certificate” including the “Letter of Exemplification”. 1. Hello and thank you for your question.
If you were born in the state of Bihar in India, you would need to contact the consulates office within your country of birth. Each country may have a different list of requirements to obtain a birth certificate. Here is a General number (with int the US) that I located that may be able to assist you further.
      Thank you,
      Nichole H.

  78. Hi,

If my daughter tries to order my birth certificate, will I be notified?

    1. Hello Brandon,

I have never heard of a state office notifying someone if their certificate has been ordered. If someone is entitle to the record and can provide all the the required information/documentation the state will process the request. If the person ordering is not on the list of entitle people then the state vital records office will deny the request for the certificate.
Each state has a list of people who are entitle to order a certificates. If she is considered entitle to your record in the state you were born then she would be able to order. If you would like to know the list of entitle people you can contact the Vital Records office with in the state you were born.
      Thank you,
      Nichole H

  79. I need to get a copy of my daughter birth certificate what do I need to take to city Hall to show proof

    1. Hello Emmy and thank you for your question.
You will need to go to the state or county vital records office with in the state & or county your daughter was born. Be sure to bring your valid government issued I.D., payment for the certificate and if your name is different now then it was on her birth certificate you may be required to show how or why your name changed.
To find out the required documents for this specific location you can visit their website or contact their office directly by phone before you make the trip to their office. If you need assistance locating their contact information please let me know.
      Sincerely,
      Nichole H.

  80. Hi! I was born in the state of New Jersey, but I now live in upstate New York. I need a copy of my birth certificate for my marriage license, but I don’t know what kind of proof of relationship I should provide. Should I write a statement, and include my SSN and birth date? Or just take a picture of my driver’s license and SSN card? Does the proof of relationship be notarized? I have already submitted an order with your company and need my birth certificate as soon as possible.

    Thank you for your time.

    Sincerely,
    Stephanie

    1. Hello Stephanie and thank you for your question.

Here is the link to order a birth certificate from the state of New Jersey http://www.state.nj.us/health/vital/order-vital/online-requests/index.shtml You should be able to find the acceptable documentation per New Jersey listed here.
      Thank you,
      Nichole H.

  82. Hi
My grandmother was born in Wisconsin in 1926 but was never issued a birth certificate. I believe she has a baptismal certificate. We would like to take her abroad for her 90 th birthday. Is there a way to obtain a birth certificate at her age?
We have other documentation, marriage certificate etc, that states she has been living in the USA as well.

    1. Hello Cyn and thank you for your question.

Yes, your Grandmother is able to obtain her birth certificate from the state of Wisconsin

      WISCONSIN BIRTH CERTIFICATES
Wisconsin Vital Records issues only certified copies of Wisconsin birth certificates, which are typically acceptable for passport, social security, employment, personal identification and other legal purposes.
      https://www.vitalchek.com/birth-certificates/wisconsin/wisconsin-vital-statistics
Who Can Order from the stated of Wisconsin
Access to copies of Wisconsin birth certificates is restricted to the following:
The person named on the record
•The spouse of the person named on the record
•A parent of the person named on the record
•A sibling of the person named on the record
•A child of the person named on the record
•A grandparent of the person named on the record
•A person authorized in writing by one of the above. The written authorization must accompany the request and the relationship of the authorizing party to the subject of the record must be clearly explained
•A person who can demonstrate that the record is required to protect a personal or property right

      Thank you,
      Nichole H.

  83. If I do not have any official form of ID, (drivers license, passport, etc.) what can I provide as a substitute for that. The reason for getting my birth certificate is to get official ID, specifically a drivers license. Any help would be greatly appreciated.

    1. Great question, thank you for asking. Many vital record agencies do allow for alternate forms of identification in cases where a driver’s license or passport is not available. Since each agency has different rules, you can send us an email with your question at Vitals.Research@vitalchek.com and our team will provide you with that information, or you can contact the vital record agency responsible for issuing your birth certificate directly to find out what other forms of documentation are acceptable. Thank you.

  84. My license is expired and I need a new one. I need anot I.D. to get a certified birth certificate but I need the birth certificate before I get a new I.D. What do I do?

    1. Thank you for your question. Since each agency issuing birth certificates have unique regulations about the documentation that is acceptable, there’s not a simple standard answer. If you’d like to contact us at 800-255-2414 or by email at Vitals.Research@vitalchek.com, and provide the location where you were born, our team will be able to determine what other documentation may be acceptable in place of an ID to obtain your birth certificate. Otherwise, you can contact the vital record agency that issued your birth certificate directly to get that information as well. Thank you.

  85. I’m trying to enroll my child in one of her fathers tribes. I need original birth certificates back to tribal enrollment (grandparent). I do not have communication with the family anymore. I would need dads, grandmas, and great-grandmas for this tribe. My baby is less than a year. Can I order them through here or the state? Thanks

    1. Hello Reese and thank you for your questions.
If you are needing to order your child’s certified birth certificate then you are entitle to it and should be able to order through the VitalChek.com online service. As far as ordering certificates for the other family members ( Dad, Grandparents & Great Grandparents) it would depend on the state where they were born. Each state has its own list of entitle people. If you have further questions regarding entitlement you can contact our office at 1-800-255-2414 and we’ll be happy to help.
      Sincerely,
      Nichole H.

  86. i paid for a birth certificate. got a confirmation number but didn’t get an email confirmation with it

    1. Hello Chris and thank you for reaching out to us.
Sometimes the email confirmation can get caught up in a spam filter and or sent to your junk mail. If you still do not see this confirmation email (from DoNotReply@vitalchek.com) please contact our office at 1-800-255-2414 and we’ll be happy to look into this further.
      Sincerely,
      Nichole H.

  87. Can I get my niece’s birth certificate for her. she is not in touch with her mom and dad and can’t get an ID without her birth certificate. she has no picture ID to fax any other ways to prove her identity?

    1. Hello and thank you for your question.
You being able to assist your niece with ordering her certificate will depend on the state where she was born. However, when ordering on the VitalChek.com site there can be other ways your niece can verify who she is without having to provide an I.D. Some states require online verification, this consists of a serious of security questions. Most states also have an option for alternate I.D.s to be provided in-case the person doesn’t have a valid photo I.D. Please feel free to contact our office at 1-800-255-2414 and we’ll be happy to answer any further questions you may have.
      Sincerely,
      Nichole H.
      Thank you,
      Nichole H.

  88. Im a rsa born citizen but i dont have a birth certificate because my mother doesnt live with me tips on how to get one

    1. Hello Nicholas and thank you for reaching out to us.
Can you clarify where you born? If you meant to say a “USA” born citizen, then as long as you are over a certain age (legal age to order varies by state) you should be able to order your own birth certificate. Please feel free to contact our office directly at 1-800-255-2414 and we’ll be happy to assist you however we can.

  91. Hii, We never filled out the birth certificate form we got from the hospital, I was scatter brained the first few months after he came home. He’s now three and since we are thinking about schooling, I started to think of where I put his birth certificate… Then bam! I realized it was never ordered.. Obviously, I feel like a terrible mother.. Not ordering my child’s birth certificate.. Since he was born in a different state, do I need to get it from there?

    1. Hello Hailey – no worries, this happens more often than you may think. You will need to order you child’s birth certificate from the state where they were born. You may also be able to order on our site https://www.vitalchek.com/birth-certificates . 92. After submitting the order for the birth certificate of my daughter, the next step asked for my proof of identity (which I have as my driver’s license), and “proof of relationship.” Is that a separate form that I need to fill out? Where would I get that from? Ordinarily, a birth certificate would probably fill that role, but as I’m ordering my daughter’s first, that’s not an option.

Thanks in advance for your help!

    1. Hello Joel and thank you for reaching out to us. Please email us directly at Vitals.research@vitalchek.com including your VitalChek order number or contact our office at 1-800-255-2414 so we can look into this and see why it is asking you for proof of your relationship to your daughter. This is typically not asked when ordering your own child’s certificate. It’s possible something was entered incorrectly on the order.
      Thank you again,
      Nichole H.

  93. I’m trying to help my fiance get his birth certificate, so that he can obtain a state identification card. He doesn’t have his social security card either.

    1. Hello Halina – If your fiancé is able to order his record through the VitalChek online ordering site then he may not need to provide I.D. Each state has a verification process set up on our site. Some states require the person to answer security questions and if those questions are answered correctly then no I.D. is needed. There is typically a list of alternate I.D’s that they will accept if someone doesn’t have a valid I.D. Here is the link to our VitalChek site https://www.vitalchek.com/birth-certificates if your fiancé would like to place his order online or he can call us at 1-800-255-2414.
      Thank you,
      Nichole H.

  94. Hi! I was curious if I need my shot record to receive my birth certificate. Also, can a 16 year old get a birth certificate by themselves at Detroit?

    1. Hello AJ and thank you for your question.
For the state of Michigan they will allow you to order your own certificate if you are over the age of 15. In most cases the person ordering will have to provide I.D.
      Thank you,
      Nichole H.

  95. Can a 17 year old get their own certificate without a parent and what documents are required

    1. Hello Roquliea,

It would depend on the state where the 17 year old was born. Some states will allow someone as young as 13 to order their own birth certificates and others require the person to be at least 18 years of age. To find out the age requirement for a specific state please let us know the location and we’ll be happy to look into this further.
      Thank you,
      Nichole H

  96. My daughter is 2 years old and I never went to get her birth certificate. Is there any special paper work I need to fill out or can I just order one online? Her father never put his name on her BC because the law in Ohio states that if the couple is not married the father needs a photo ID and at the time of her birth he did not have that. He now does and we would like him on her BC. How do I go about this in order to make sure I not only have her BC but that her father is also on it?

    1. Hello Ms. Cvelbar and thank you for your question. The following link will provide you with valuable information about the process for amending a birth certificate in the state of Ohio as well as a contact phone number for any further questions you may have about the process. http://www.odh.ohio.gov/vitalstatistics/legalinfo/leglname.aspx

  97. Hello! I live in New York, and I’m going to be changing my last name soon. I’ve seen on the New York City Civil Court website that if I were to submit my original birth certificate for proof of identity purposes, I may not be able to get it back. Preferably, I would like to have my original birth certificate on me, should the need for it arise. Can I ask for it back after, or should I just play it safe and submit a certified copy?
    Thanks!

    1. Hello FI. Thank you for your question. If it is important to you to hold on to your original birth certificate, we suggest you order a certified copy to submit with your name change request, especially since the court’s own website cautions that you may not get it back. Online ordering for certified birth certificates is available at VitalChek.com.

I am trying to obtain a copy of my birth certificate, as I misplaced mine and need it to start a new job. I did it through vital records, however, they keep rejecting the documents. The problem is my legal name is Alexander but everything(including my government ID says Alex. Is this the reason they are not being accepted? I have supplied several documents from the sheet they provided but it is still getting declined. Any advice?

    1. Hello Alex. Thank you for your question. Its is possible that the rejection of your documents is due to the discrepancy in your name. We recommend you contact Vital Records to find out what steps you need to take to resolve the issue.

  108. Can a father order a copy of a birth certificate if he was never listed on the original?

    Thanks

    Kim

    1. Hello Kim. Thanks for reaching out with your question. While each state has different guidelines for who is eligible to receive copies of vital records, it is unlikely the father would be able to order a birth certificate he is not listed on unless he has appropriate legal documentation proving paternity. We recommend you check with the Vital Records agency in the state where the child was born for more information.
